INSTRUCTIONS

  1. Fill out this form and submit to the nearest SSS branch office together with the required documents.
  2. Fill out the applicable portions as follows:

Parts I-A, B and D, if applying for SS number as pre-employment requirement
Parts I-A, B, C and D, if applying for Self-Employed, Overseas Filipino Worker (OFW) or Non-Working Spouse membership

REMINDERS
For Part I-B “DEPENDENT(S)/BENEFICIARY/IES”, use “Additional Sheet for Dependent(s)/Beneficiary/(ies)”, if necessary.

  1. Always indicate “N/A” or “Not Applicable”, if the required data is not applicable
  2. If this form is to be downloaded from the internet, please fill-out in two (2) copies.

REMINDERS

New registrant who is over sixty (60) years old and not a surviving spouse pensioner/guardian of a pensioner, is not qualified to apply for an SS number.

  1. Your SS number is your lifetime number. You should not have more than one SS number.

The following required documents should be the original or certified true copy issued by the City or Municipal Civil Registrar or Philippine Statistics Authority/National Statistics Office:

LIST OF DOCUMENTARY REQUIREMENTS

Always present the original or certified true copy/ies when submitting the photocopy/ies of the required ID card(s) and/or document(s).
A. Change of membership type

  1. To Self-Employed – No required documents
  2. To Non-Working Spouse – Marriage Contract/Marriage Certificate or a copy of Working Spouse’s Member Data Change Request form (SS Form E-4) duly received by the SSS where the name of the NWS is indicated as the spouse
    B./C. Correction of name and/or Correction of date of birth 1. Birth Certificate or Passport.
  3. In the absence of the Birth Certificate and Passport, the following are the required ID cards and/or documents:
    a. Certificate of Non-Availability of Birth Records from the City or Municipal Civil Registrar or Philippine Statistics Authority/National Statistics Office or National Archives, for the alleged correct name/date of birth; and
    b. Any TWO (2) of the following, both with the correct name and at least one (1) with date of birth:

  4. Required additional ID cards and/or documents for the following cases:
    a.If for correction of date of birth and submitted birth certificate is registered after the 55th birthday – two (2) ID cards and/or documents in Item 2.b above.
    If for correction to totally different name/middle name (except if due to naturalization) – Joint Affidavit of two (2) persons who have personal knowledge of the facts and circumstances on the use of the different name/middle name stating therein that the two (2) names refer to one (1) and the same person and the reason why the name was used.

SSS Unified Multi-Purpose ID (UMID) Card Application Form

INSTRUCTIONS

  1. Fill out this form in one (1)  copy.
  2. Erasures/alterations are not encouraged. However, if necessary, susch will be limited up to two (2) erasures/alterations only. Always affix initials on all erasures alterations of this form.
  3.  Place a check mark on the application box.
  4.  Always indicate “N/A” or “Not Applicable”, if the required data is not applicable.
  5. Indicate the home address. If permanent home address is in the province but working in Metro Manila during weekdays or working abroad, indicate the provincial address instead of the Metro Manila address.
  6. Write the “HEIGHT” in centimeters and “WEIGHT” in kilograms.

To convert: 1 feet = 30.48 cm   1 in = 2.54 cm 1 lb= 0.4 536 kg.

  1. Limit the distinguishing features is to those that can be found on the face such as “a mole under the right eye” and “mole or birthmark on the left cheek/forehead”.
  2. Always indicate the following mandatory information:
  • Country of place of birth, if born outside the philippines
  • Mobile number, if applied locally*
  • Email address, if applied abroad*

*If card applicant cannot provide the required mobile number/email address. Indicate the card applicant’s immediate family member’s mobile number/email address where SSS can communicate with the card applicant.

  1. For all types of card replacement, pay the required fee at any SSS branch office/accredited bank/collecting agent. Write the Special Bank Receipt (SBR)/Receipt Number/Transaction Reference Number on the field on the field provided and submit this form together with a required document/s and proof of payment to the nearest SSS branch office.
  2. For card replacement due to unclaimed UMID cards beyond five (5) years, a replacement fee and biometric data re-capture is required.
  3. Submit this form to the nearest SSS branch with the following required documents (use the table Documentary Requirements Guide).

SSS Kasambahay Unified Registration Form

INSTRUCTIONS

A. Register to any of the Agencies (Pag‐IBIG/PhilHealth/SSS).
B. Submission of Documentary Requirements* Not yet needed at the time of registration for the issuance of Pag‐IBIG/PhilHealth/SSS Numbers.* If not available at the time of registration, registrant will still be provided the corresponding numbers. However, availments of any benefits shall only be allowed upon submission of documents to prove his/her identity and payment of required premium contributions.

C. List of Acceptable Documents and Conditionalities:
For SSS Only

  1. Primary DocumentsSubmit photocopy & present original/certified true copy of any of the ff:
  • Birth Certificate
  • Baptismal Certificate
  • Drivers License
  • Passport
  • Professional Regulation Commission (PRC) Card
  • Seaman’s Book
  1. Secondary Documents
    In the absence of Primary Documents, submit photo copy and present original/certified true copy of TWO (2) of the following, BOTH should
    bear the name and at least ONE (1) should indicate the Date of Birth:
  • ATM Card
  • Bank Account Passbook
  • Birth/Baptismal Certificate of Children
  • Marriage Contract
  • NBI Clearance
  • Police Clearance
  • Postal ID Card
  • Voter’s ID/Affidavit
  • School Records
  • ID Card issued by Local Government Units (e.g. Barangay, Municipal/City)

If the required supporting document/s is/are not available at the time of registration, or if registration is done at Pag‐IBIG/PhilHealth, or if the
Registrant is unavailable to sign the document, SS Number shall still be issued, subject to the following conditions:

  • Membership Status of Kasambahay is “Temporary”.
  • The SS Number issued can only be used for contribution payment and
    employee reporting (by the Household Employer).
  • Submission of Primary or Secondary document/s and/or signature in the
    Form is required for conversion of Membership Status to Permanent,
    thru Member’s Data Amendment Form (SSS Form E‐4)
  • Availment of SSS Benefits and Loans is only allowed for Permanent
    Membership Status, subject to qualifying conditions.

D. Updating/Change in Personal Information, Dependents/Beneficiaries should be submitted to each agency (Pag‐IBIG/PhilHealth/SSS).

E. This form is not applicable for Family Driver. Registration of Family Driver should be done in each agency (Pag‐IBIG/PhilHealth/SSS).
